TERMS AND CONDITIONS OF USE

Cone Health is pleased to offer information about MyChart, which provides you online access to your health information from our electronic medical record. We believe you should know how we handle information we receive and send electronically. These Terms & Conditions of Use outline our practices and sensitivity to your right to privacy. If these conditions are violated, we will notify you prior to further action, which can include limiting your MyChart access to “View Only.” Note: Cone Health reserves the right to revoke access at any time for any reason.

Response to Electronic Communication - Cone Health will make its best effort to provide a timely response to electronic inquiries. Please do not send messages through MyChart that require immediate attention or which involve emergency/urgent medical issues. Clinic staff or providers caring for patients will not be immediately available to respond to messages sent via MyChart. Please call the practice office, dial 911 or head to your closest emergency department for urgent or emergency medical issues. Please allow at least three (3) business days for a response to messages sent via MyChart.

Furthermore, with respect to any electronic communications sent by the patient, we are only able to respond to such communications based on the information provided to us.

Web site Links - MyChart may offer links to related medical websites not managed by Cone Health. These website link(s) are for informational purposes only. Cone Health does not endorse and has not verified the accuracy of the information in/on these websites, and you should not rely on any of the information found on the websites for purposes of treatment or diagnosis.

Minor Eligibility - Parents and guardians can request electronic access to their child's medical record until the child's 12th birthday. At that time, a new request must be submitted if parents and guardians choose to provide consent for an adolescent ages 12-17 to have access to their own MyChart account.

Once enrolled in MyChart, adolescent patients can:

  • View certain test results.
  • Request appointments.
  • View allergies, medications and immunizations.
  • Send messages securely to your trusted care team.

Parents and guardians can apply to have access to the adolescent's account to:

  • Request appointments.
  • View allergies and immunizations.
  • Send messages securely to the care team.
  • View family history and status.

Note: Adolescents will not have direct access to family history information documented in their electronic medical record until their 18th birthday.

E-mail Privacy - Patients who are users of MyChart should be aware that they will be notified via email when there is new medical information to be viewed in MyChart. This means that any person with access to a patient’s email will be able to see this notification. This could include the patient’s spouse, employer or anyone else that can access a patient’s email account. Although no private medical information will be sent, the notification that new medical information is available by accessing MyChart may be information that a patient would not want others to know. Thus, patients should take this into account when providing an email address.

Please know that if you send us an email communication, it may be shared with Cone Health staff who assists the physician in providing the patient’s medical care. A patient’s confidential medical information on MyChart will be accessible only to appropriate clinical staff.

Security and Confidentiality - We afford the same degree of confidentiality to medical information stored in MyChart as is given to medical information stored by Cone Health in any other medium. Cone Health is committed to protecting the confidentiality of your medical information. We limit Cone Health employees' access and ability to enter or view information based on their role in your care. Firewalls, passwords, encryption, and audit trails are further used to safeguard your information. We shall identify the records released and note the time and date of access each time a patient accesses MyChart. We have taken steps to make all information received from our online visitors as secure as possible against unauthorized access and use.

For other than general information viewing, MyChart must be accessed with a Secure Sockets Layer (SSL) compatible browser or terminal(Chrome or Internet Explorer versions 10.0 or greater). Our SSL web server uses authentication and offers the highest level of encryption technology commercially available(128-bit RC4).

You can tell when you are secure by looking at the location (URL) field. If the URL begins with https:// (instead of http://), the document comes from a secure server. User names and passwords provide two layers of authentication and are stored in an encrypted database that is isolated from the Internet. As a MyChart user, your role in maintaining the security of your medical information is: 1) Changing your password on a regular basis, and 2) Keeping your login ID and password confidential.
